Update On 37 Military Hospital Post Diploma Top-Up Application Forms 2024/2025

GENERAL ENTRY REQUIREMENTS

• 1. Prospective applicants must be 16 to 35 years of age.
  2. Prospective applicants must have credit or better grade (A1-C6) in both core subjects (English, Mathematics and Integrated Science) and three (3) Electives subjects
  3. Eligible applicants must have a Biometric Birth Cert, Valid Ghana National Identification Card and National Health Insurance Scheme (NHS) Card.
  4. A combination of SSSCE and WASSCE results are not acceptable
  5. Applicants who are above 35 years Must Be Serving officers with letters from employers.

DOCUMENTS NEEDED 

You will need the following documents to complete the application process:

1. 1. Passport picture (size of picture should not be more than 2M)
   2. Your Educational Certificates and Transcript must be scanned and saved in pdf format

• Visit any CBG branch nationwide to purchase the application form (e-voucher) at a cost of 200 GHS.
• Now log to the admission portal via this Link
• Enter the e-voucher serial number and PIN, and complete the form by providing the information below.
